Member Rights Policy

You have a right to receive information about EviCore, its services, practitioners and providers.

You have a right to be treated with respect, with recognition of your dignity regardless of your race, sex, religion, age, or sexual orientation and you have a right to privacy.

You have a right to participate with your practitioner’s decision making regarding your health care.

You have a right to candid discussion of appropriate or medically necessary clinical service options for your condition, regardless of cost or benefit coverage.

You have a right to voice complaints or appeals about EviCore or your Health Plan. You can contact your health plan’s member services for help.

You have the right to expect a cooperative effort between EviCore and your health plan so you have the opportunity to deny or authorize the release of your personal health information. You have the right to expect that personal health information maintained in EviCore systems will be used only for treatment, payment or health care operations and that your permission is required before releasing confidential information outside of EviCore.

You have the right to make recommendations regarding this member rights policy or to receive a copy by calling EviCore Member Services at 800-420-3471.

You have the responsibility to supply information (to the extent possible) that EviCore or its referring and rendering providers need in order to provide care or determine appropriate services.

Notice of Availability of Auxiliary Aids & Services

EviCore healthcare is committed to making its information and technology accessible in accordance with Section 1557 of the Affordable Care Act.  EviCore Healthcare provides various auxiliary aids and services, including alternate formats, at no cost in order to better communicate with people with disabilities.
