Effective October 1, 2024, providers submitting for medical drug prior authorization for Aspirus Health Plan members will go through http://www.EviCore.com to build the case on the web. Care Continuum (CCUM) will manage the Medical Specialty Drug program and utilization review, and EviCore.com will act as a single sign on portal, allowing the provider to submit an electronic request to CCUM. Case initiation can also be initiated on the phone through CCUM at 866-540-8289 or faxed to 866-540-8935.
