Except for Proton Therapy requests, effective for dates of service after January 1, 2026, EviCore by Evernorth will no longer process prior authorization requests for BCBS Medicare and Healthy Montana Kids members.
Beginning in October 1, 2025, EviCore by Evernorth will handle the review of prior authorization requests for Proton Beam Therapy for Blue Cross Blue Shield of Montana Medicare Advantage members.
