
Fallon Community Health Plan Provider Resources
Please download and read through the following material. Our goal is to make the change to EviCore as seamless and easy as possible and these documents will explain the process. There is contact information in the material if you have any questions.
Effective 10/01/2025, the management of Fallon Health prior authorizations through EviCore will transition from the MedSolutions portal platform to the CareCore National portal platform. The User ID and Password will remain the same.
EviCore is pleased to announce that we are expanding our relationship with Fallon Community Health Plan for benefits management of covered imaging services performed on behalf of FCHP members.
Prior-authorization is required for included MR, CT, PET & NCM services with dates of service January 1, 2010 and thereafter. MedSolutions will accept authorization requests beginning Friday, January 1, 2010.
To aid the transition, EviCore and FCHP will be hosting web-based orientation sessions to provide your staff with instructions for how to be successful in the program. MedSolutions encourages attendance at a convenient session. You may find the schedule below.
FCHP has recently mailed an announcement letter to providers in the service area describing the program. Please review the documents for more information. If you have any questions about this new program, please call Fallon Community Health Plan at 866-ASK-FCHP (866-275-3247) or EviCore healthcare’ Customer Service Department at 888-693-3211, option #3.
