Health Alliance Plan MI Provider Resources
EviCore by Evernorth is pleased to announce its partnership with Health Alliance Plan (HAP) to provide authorization services for HAP members enrolled in Medicare, and Commercial plans for dates of service beginning June 1, 2011. Prior authorization will be required for selected Cardiac Imaging, High-Tech Radiology, Musculoskeletal Procedures, Radiation Therapy, and Sleep Studies. Services performed in conjunction with an inpatient stay, 23 hour observation, or emergency room visit are not subject to authorization requirements.
