Health Choice Arizona Provider Resources
The radiology program will include membership from both the Arizona Medicaid Program and Health Choice Generations (Medicare). The program will become effective for dates of service beginning June 1, 2010. EviCore will begin accepting authorization requests on May 24, 2010.
