Effective 10/1/2026, EviCore will cease processing for Lab Management prior authorization requests on behalf of Highmark. For any prior authorization requests submitted on or after 10/1/2026, please submit via the Highmark provider portal.
Effective May 1, 2026, EviCore will no longer process Musculoskeletal Pain Management, Joint and Spine Surgery prior authorization requests on behalf of Highmark. Starting May 1, please submit all prior authorization requests for these services directly through the Highmark Provider Portal.
Effective December 2, 2025, EviCore will no longer process Radiology and Cardiology prior authorization requests on behalf of Highmark. Starting December 2, please submit all prior authorization requests for these services directly through the Highmark Provider Portal.
