Humana Healthy Horizons in Kentucky Provider Resources
EviCore healthcare is pleased to announce its partnership with Humana Healthy Horizons® in Kentucky to provide authorization services for Kentucky Medicaid enrollees. For dates of service beginning March 1, 2021, EviCore coordinates prior authorization for selected radiology services. We are pleased to announce that beginning with dates of service on or after Sept. 1, 2022, EviCore will coordinate prior authorization for physical therapy, occupational therapy and speech therapy services.
