EviCore healthcare is pleased to provide prior authorization services for Summit Health for dates of service beginning January 1, 2021. Prior authorization will be required for the following services:
• Advanced Imaging
• Cardiac Imaging
• Interventional Pain
• Spine Surgery
• Joint Surgery
