Banner Health Provider Resources

EviCore is pleased to announce we are expanding our partnership with Banner Health Network to provide authorization services for Banner members enrolled in Medicare and Medicaid plans for dates of service beginning April 1, 2022. Prior authorization will be required for selected programs:

• Radiology
• Cardiovascular
• Radiation Oncology
• Medical Oncology
• Musculoskeletal
  • Interventional Pain Management
  • Joint/Spine Surgery
  • Physical Therapy
  • Occupational Therapy

Since 2018, Banner members have received pre-service authorization for CT, CTA, MRI, MRA and PET procedures from EviCore healthcare.

Effective May 1, 2019 additional services requiring pre-service authorization were added for AARP Medicare Complete (United Healthcare) to include the following:

• –Cardiovascular 
• –Echo
• –Stress Echo
• –Nuclear Stress
• –Diagnostic Heart Cath
• –Cardiac CT, MR, PET

Effective July 1, 2019 services requiring pre-service authorization were expanded to include the following:

• Radiation Therapy
• Medical Oncology
• Musculoskeletal
  • Interventional Pain Management
  • Joint/Spine Surgery
  • Physical Therapy
  • Occupational Therapy
