EviCore by Evernorth is pleased to announce its expanded partnership with Moda Health to provide services for members enrolled in Moda Health programs.
Effective May 1, 2026, Moda Health Fully Insured and in-scope Self Insured members in Oregon, SW Washington, Idaho and Alaska will require prior authorization for Lab Genetic Testing services through EviCore for dates of service beginning on or after May 1, 2026.
Effective June 1, 2026, Moda Health members in Texas require prior authorization for Lab Genetic Testing services through EviCore for dates of service beginning on or after June 1, 2026.
