Clover Health Provider Resources
EviCore by Evernorth is pleased to provide prior authorization services for Clover Health for dates of service August 1, 2020 and beyond. Prior authorization will be required for the following services:
- Advanced Imaging
- Cardiac Imaging
- Radiation Therapy
- Musculoskeletal - Interventional Pain, Spine and Joint Surgery
- Sleep Covered Services and Related Equipment
Please refer to the CPT Code List located within the General Resources tab for procedures that require prior authorization through EviCore. If the procedure isn't found on the CPT Code list, please review authorization requirements through Clover Health's web portal:
Effective 11/1/2021 All part b drug authorizations for Clover Health will be submitted to CVS-Novologix. You may submit an authorization request to CVS-Novologix through the following channels:
