There are three methods of requesting a prior authorization.

 

The portal is the most efficient and preferred method to request authorizations. Not only does it save time, it is available 24/7. Access www.evicore.com .

For web portal training: register at http://evicore.webex.com/

 

Phone is available primarily for questions but a prior authorization can be initiated by phone. This process does take longer than the portal.

All outpatient elective MR, CT, PET, and Nuclear Cardiac studies performed on or after December 1, 2013 will require prior authorization from EviCore healthcare.
