As of 1/1/23 , EviCore will no longer manage new authorization requests for AdventHealth Plan members. Please contact the health plan at the number on the back of the member’s ID card.
EviCore healthcare is pleased to partner with AdventHealth Advantage Plans to provide authorization services for members enrolled in Medicare and Commercial plans. As of 1/1/22, prior authorization will be required for the following services: Radiology, Cardiology, Medical Oncology, Laboratory Management, Sleep Management, and MSK Pain.
